Primary Care Discharge Liaison Service

Working closely with our colleagues at Frimley Health NHS Foundation Trust, EBPC provides a very experienced GP to the acute hospital to ensure discharges are optimised. The GP liaises between the acute hospital team and Primary Care teams in the community to ensure normally unregistered patients are discharged with access to GP follow up and registration at an appropriate local practice.
